Adult’s Social Worker
Team: Hospital Discharge
Location: Manchester City Council
Details:
There is a 3 month contract based within the Hospital Discharge to assess community team based in North Manchester at Harpurhey District office. The role is hybrid and applicants can work flexibly between the hours of 8-6.30. There is weekend working approx 1 in 4/5 and you take days back in the week.
The applicant will be able to work in a fast paced team assessing people who have been discharge from hospital and completing strengths based assessments for packages of care. The candidate must have experience working with adults and be able undertake capacity and best interests decisions. The person will be able to work within a multidisciplinary team and be a good team player
Hybrid working. Based at Harpurphey district office flexible working between the hours of 8-6.30. Weekend work

• DIPSW or equivalent qualification in Social Work
• Social Work England registration
• Enhanced and in date DBS certificate.
• Excellent communication skills and be able to make good decisions based on information available.
• Can work alone and within/contribute to a team/multi-agency set ups
• Possess knowledge of service needs.
• Competent IT skills and be able to navigate electronic file/recording systems.
• Can write clear, concise, and robust assessment/reports/files.
• Have a flexible approach to work and working hours when the need arises
