*** 6 X Adult Social Workers needed – Various Teams – Blackburn – £30 ***
3 x Adult Community Social Care Posts
As a member of a multi-disciplinary, multi-professional team the Social Worker will effectively manage an agreed caseload of complex cases in accordance with Blackburn with Darwen Borough Council Policies and statutory legislation including assessing and establishing needs and identifying options / solutions or signposting to colleagues other services to meet those needs. The posts will be located in three separate well established integrated neighbourhood teams within Blackburn and Darwen. The role involves undertaking social care assessments, s.42 safeguarding enquiries, initial reviews, annual reviews, completing capacity assessments and best interest meetings.
2 x Adult Community Social Care Posts – Short Term Care Reviews
As a member of a multi-disciplinary, multi-professional team the Social Worker will effectively manage an agreed caseload of complex cases in accordance with Blackburn with Darwen Borough Council Policies and statutory legislation including assessing and establishing needs and identifying options / solutions or signposting to colleagues other services to meet those needs. The posts will be located in three separate well established integrated neighbourhood teams within Blackburn and Darwen. This role includes completing short term care reviews following discharge to assess placements, this role will include completing reviews, capacity assessments, best interest decisions, supporting people to return home in a safe and timely manner.
1 X Hospital Social Worker
The role is varied, rewarding and as a team we focus on the Home first approach to discharge. As a Social Worker on the team, you would be supporting with triaging of referrals with our Homefirst and Reablement team colleagues, undertaking a duty role and working with people throughout their journey from hospital or rehabilitation. We are looking for an individual who works well with multi agency professionals, thrives in a fast past setting, is flexible to operational demands and works well under pressure
