Team: Immediate Tier Services (Winter Pressures)

We are looking for a Social Worker to work within the Intermediate Tier Services to support with Winter Pressures and vacancy deficit. This service covers Admission Avoidance; Intermediate Care and Discharge to Assess bed-based units.
We are looking to for an enthusiastic and motivated individual who has a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence.
As a Social Worker in IMC you will work in one of the designated services and undertake assessments/reviews of need under the Care Act 2014, assist with support planning as necessary and also link people in to universal and voluntary services. You will also assist colleagues within other areas of the services as necessary and support with complex assessments, mental capacity and best interest assessments
