Based at Michael Carlisle Centre and across SHSC Trust work sites. The candidate will need to be an experienced Social Worker with a mental health background.

Core tasks and duties as follows:

– Undertaking social care assessments and delivering timely care packages for hospital inpatients prior       to discharge
– ome mental capacity assessments, BI decisions and safeguarding as required
– ttending 117 meetings, ward rounds, professional meetings
– Facilitating learning opportunities for ASYEs through shadowing
– Participation in duty rota
